3-16-07 Tommy was at Mom's for a year. There was a nursing staff that helped Carolyn. Tommy did great at home. He went to doctor visits and hospital for check ups, but was not in hospital for any extended time. The family is very proud of Carolyn for the great job she has done.

In January 07 Tommy did go to the hospital and was there for about a month before he went to a nursing home and was there only a couple of weeks before he went back to hospital and was very sick again. The Doctors said Tommy has blood clots on his lungs and other infections that they can not figure out where or what they are. All the antibiotics and meds are highly toxic which are bad for him and will start affecting other organs.

Tommy has been a pin cushion for 2 1/2 years. Little Tommy said he did not want his Dad poked no more. It makes him sad to see all the bruises. The Doctor has suggested that Tommy go to Hospice, the antibiotics are doing more harm than good. Carolyn has told all of Tommy's brothers and sisters and the family what the Doctor has said. On Friday March 16, 2007 Tommy was moved from Luthern Medical to Collier Hospice. Tommy is resting and getting such good care.

Tommy, Tina and the kids had visited Lakewood Christian Church before Tommy was assaulted. The Pastor came to see Tommy at Denver Health when he was first hurt. We have contacted the Pastor and Tommy is going to be Baptised.

We will keep you posted Thank you for all your thoughts and prayers.

PEACE BE WITH YOU!!!!!!!!!!

Thomas Lee Martin was assaulted 10/10/04. He was taken to Denver Health at approx. 4:30 am. for severe head trauma. He was taken into surgery to relieve pressure in his brain and stop the bleeding. They removed his left bone flap. He is in critical condition. There is a rod in the right upper side of his forehead that measures the presure on his brain.

Tommy is very sick they say he wont make it thru this. One of the worst cases of head trauma they have seen in a long time. After surgery they done a cat scan there is minimal brain activity.

Updates

Tommy is on a ventilator and is heavily sedated.  Has normal blood pressure, oxygen level.  He is being fed thru the nose.

10-12-04
They have done another cat scan and tommy has had 3 strokes and brain has shifted. ICP levals are high 27/31, when laid flat they go higher.

10-13-04
They have removed the tubes from the nose they are in his mouth.  He has a sinus infection and a sore in his nostril.  At this point he is on Fentinol , Versed, Insulin, levaquin .  Mannitol to release the pressure on the brain every 3 to 6 hours.  He has started to dehydrate because of the mannitol.

10-14-04
They have inserted the tube further into tommys brain to drain the fluid.  In one hour his ICP level had dropped to 13/17 at night as low as 11.
10-16-04
Tommy has a temp of 37 to 38.  There are signs of pneumonia.  So they have started a universal antibiotic.

10-17-04
They have tried to take Tommy for x-ray of the spine, his ICP numbers were rising high so they decided not to take him.

10-18-04
They have done at cat scan today and x-ray.

10-19-04
All numbers are holding the same.  Signed for them to do a trach and feeding tube.  Oxygen at 50%.  Temperature running a little low.  Took off the cold blanket and covered him.

Nurse said people are calling to get information and they cant give info over the phone. Some of the receptionest check people out others dont they called Carolyn to check if it was ok for Karen to go in.

4:00 P.M. BP 118/57 ICP 18/20 vent 32/35 oxygen is 88%. Tommy is back fron x-ray. The nurse Bailey and respitory backed Tommy off the ventilator He was breathing on his own. They were both very surprised. Good job Tommy. His reflex in feet and hands are good. He was pissed when she checked his nails, he pulled back like he was gonna hit her.

Right lung has a shadow. Vent is 32/34/35 oxygen is set 55 92% HR 122 ICP 20/23 BP 119/54. Start watching number in parenthases after BP take that number and subtract ICP number. It needs to be above 50 would like it at 60. As far as we can remember the number has been good, has run about 60/65. Dont know what it is but nurse Bailey says it is very important. Bailey explained that because the bone flap is out there is no danger of fluid going down his spine. (cistes space) Which is what they do cat scans for. Tommy's cat scan shows there is plenty of space. Bailey is concerned that Tommy get his trach monday because his lungs show signs of damage, due to fluid build up. Because of vent in the mouth they are not able to suction deep enough, with a trach they can go deeper and get the fluid out. She said to push like HELL and demand it get done. If you have to ask to talk to chief of surgery.

2:45 P.M. HR 114 ICP 21/23 BP 113/56 (76) oxygen 55 @98% vent 32/32/34 they had moved him from right side to left.

She says his numbers are good and holding steady. They wont take off vent because his ICP # are a little high. Respitory says it would be a good idea for him to be trach as soon as possible.

HR 106 ICP 14/17 temp 36.3 BP 119/54 (75) vent 32/38 oxygen 55 @92%  Tommy looks real good, not having a hard time like yesterday.  His fentinol is down to 6 ativan at 1.  Chest tube is doing its job at 80 now. Set on water seal.  As long as he does not have any more problems the plan is to take the chest tube out in 24 hours. They are weaning Tommy off the meds.  They are talking about moving Tommy to an actue care facility .  But first he needs to be off meds and rod out of head which should be by end of the week.  Social worker megan gave paper work on North Valley Rehab Hospital and said he could be ready to move by the first of the week November 1, 2004

Spoke with Dr. Sinclair and seen lung xrays. Right lung is very shadowed. She said they were going to do an ultrasound to see if it was fluid, if it is they would have to move the chest tube to a different spot. Nurse cecilia said she called doctor in to look at eyes when we left last nite she is sorry but made a mistake by telling us he had movement.

8:35 P.M. HR 132 ICP 23-24 temp is 38.4 BP 136/58 (82) oxygen is 94% @ 55 vent is 16/23 chest drain 183 Tommy's numbers are a bit high because they just brought him back from a cat scan. Asked about the chest drain amount, and nurse said they didn't turn it on, it's just been on water seal which allows fluid to drain and with moving him around it for the tests must have made it drain. She said they didn't move the chest tube. Nurse said that he was really peeing too, more then she has seen him do before. Getting rid of some of the excess fluid in his body.  Also the tube to drain fluid from the brain was taken out. They will watch the ICP numbers closely now. They are going to take him down again for a bilateral xray.

11:45 A.M. HR 135 ICP 15-16 BP 144/69 (93) Vent 16/28 Oxygen 55 @ 93% Temp is 38.8 Fentinol 2.5 Thru the nite his ICP was up to 40. He is finished with the Sufapine antibiotic but his temp is still going up. Talked to nurse Mike he said the temp is not affecting the ICP # anymore so thats not an issue. They took cultures of blood, urine, spit and chest fluid to see if they grow anything and could be the cause of the temp. Results of bilateral xray came back and looked good so they took off his collar. They put in a new chest tube in a different spot. Chest drain 30.

2:40 PM HR 144 ICP 21 Temp 39.9 BP 132/63 (85) Vent 16/32 Oxygen 55 @ 96%

His hands and feet are swelling alot. He was gagging quite a bit and they have to suction to keep mouth and trach clear.

5:30 PM  HR 144 ICP 20 Temp 39.8 BP 145/69 (93) vent 16-25 Chest tube 65

9:20 PM HR 138 BP 130/58 (82) Temp 38.9 Vent 16/24 Oxygen 55% @ 96% CVP 272 which reads high because they have fluid going in, his normal reading is 16 which is good. Fentinol still at 2.5. Chest tube still at 65. They put him back on Vancomison and Sufapine antibiotics and moved the ice blanket underneath him instead of over him. Temp is finally starting to go down. Notice, no ICP #.....the rod is out!!! Tommy is looking much better tonight. They cleaned his mouth and teeth really good and used mouth wash.

2:00 P.M. Tina and Lawana was up to see Tommy today, He is running a little bit of a fever.  They done a cat scan this morning of lungs, both lungs have fluid around them.

They havent heard on cultures yet still taking antibiotics.  Surgeon said that nothing is changed with head injury, There is dry blood in the head one reason why so swollen and probably wont go down. If Tommy ever walks will probably build a plate for head or will have a helmet to protect his head.  He also said that they can't really measure the brain activity but that a good portion of Tommys is damaged.

10:00 P.M. HR 97 BP 142/71 (92) Temp 35.7 was off of vent for 3 hours earlier today  has been off vent for a little over hour now.  As long as his heart rate dont go up they will keep him off.

Nurse Suzanne also gave him 5 mg of Haldal, she said he was a little agitated and not sure if it was the brain injury or pain reaction but it calmed him down. They pulled his chest tube out a bit, seems to be working better. The head trauma doctor has released him to be able to go to acute care, just have to wait for the doctor in charge of his lungs to give the ok. He must have had a nice bath, because he smells as good as a baby. Nurse Suzanne was going to give him a shave.

11-2-04

12:00 PM HR 135 CVP 14 BP 111/51 (72) Temp 39.3 Vent 16/36 Oxy 60 @ 90% Chest drain 200 Fentinol 5. He is on Levaquin antibiotic. The doctors assistant came in and said that they wanted to run a scope to look around Tommys lung, there might be a pocket of fluid the chest tube can't get to. If they see anything they will try to break it up and suction it out. If they can't do it then they will need to make an incision, remove the bottom rib and take out the infection that way. The surgery is a risk.  But he already has the trach and vent. The cultures are starting to come back showing Psuedomonis and staph infection. Tommy's fever is still pretty high and so they just have a towel covering him and a fan going in his room to try to cool him. There is something wrong with the thermostat and they can't get the room cooler then 70 F, they are calling maintenance to look at it.

1:30 PM HR 137 CVP 18 BP 129/73 (92) Temp 39.1 Vent 16/31 Oxy 70 @ 94% They have changed the antibiotic to Vancomison. The room is cooling a bit more now. Spoke with the attending doctor and he says the decision to do the surgery hasn't been made yet, it's up to the surgeons. He also repeated that Tommys brain injury is very severe.

3:00P.M. Ambers sister and mother inlaw and another lady from there church went to see Tommy. For a prayer.

5:00P.M. HR 117 CVP 22 oxygen 60 @98% BP 122/76 (95) vent is 16/ 27 coughing alot  chest tube is out.  White blood count is in the 30's not sure what that means but Terry wanted to know.  Fentinol is at 5 Temp is 38.1  It is very cold in the room to help with his temp.  They only have a sheet across his mid section.  I talked to nurse Bailey and ask her how his cat scans were on his brain, she is very concerned the brain has 2 butter fly shaped parts in it and they are suppose  to be aligned, his are not the brain keeps shifting and the swelling is going out of the skull. This is not a good thing.

9:00P.M. Temp is down 35.7 HR 105 seems to be resting.

11-3-04

12:00 PM Not much change in Tommys stats this morning. He's coughing more. Temp is coming down. They now have a sign on the door. You have to put on a gown and take precautions before you leave his room. He now has MRSA which is Methicillin Resistant Staphylococcus aureus . It's contact contagious so we have to becareful not to take it out with us. They still have surgery scheduled for tomorrow.

3:00P.M.  Everything still pretty much the same.

5:00P.M. Doctor called said surgery will be thurs. mid afternoon.

11-4-04

12:00PM Tommy's stats are again pretty much the same, he seems to be resting calmly. The nurse said the surgery is still on for this afternoon.

2:20 P.M.  HR 120 BP 107/64 vent 16/22 oxygen is 94 @40%seems to be resting hard.  Ansthesiaologist came in and was making him cough real hard.  They say breathing is real hard.  Not sure if they should do the surgery,  Called the doctor.  Doctor came in they talked it over they believe every thing will be ok.  Gave him 2 shots of fentinol and 1 of versed.

5:30P.M. Tommy is out of surgery looks pale and more swollen. HR 128 oxygen 96 @60% Temp is 37.3 BP 134/64 (89) Fentinol is at 5  Vent is 20/26 3 chest tubes 110 cc  55 cc  Doctor will call us to tell us what happened in surgery.

8:30 P.M. Still no call from the doctor. Don't think this Will nurse knows too much of what went on temp 38.2 BP 107/57 HR 107 seems to be resting.

10:00P.M. Doctor said there was a lot of pus in there so much that they couldn't use the scope, they did clean out as well as they could but didnt do all they had planned on, Tommy started to have a hard time so they had to close him up.  Seems maybe the fluid from the brain might be moving down, this is where the fluid is coming from. Said Tommy is very sick and his brain injury is getting worse.

He said they did get quite a bit of infection out before Tommys organs started shutting down and they had to close him back up. Talking with him made Mom feel much better.  They took cultures again but will be 24-48 hours before they get the results back.

11-8-04
11:30AM We went to see Tommy but stopped and talked to the nurse and then Dr. Campsen. He said that Tommys lungs are better, but still some fluid around them. He wants to put in a new picc line in Tommys arm. The one in his chest is getting old and if they can get one in the arm it will last longer. We only got to see Tommy for a few minutes before they shoo'd us out so they could move him to the bed and start the picc line. Megan the social worker wanted to talk with Mom and took us to the family room along with another doctor who was with the ethics board. She wanted to know how realisticly we were looking at Tommy's condition. Again it was stated that he may never get any better then he is now, which we know, but she even agreed nobody can be 100% sure what the outcome will be down the road. Doctors are discussing whether to put a shunt in his brain to drain to his gut. They aren't sure if it will help him or hurt him. She also went over the areas of the brain that have been damaged, and was quite good at explaining in a way we could understand. She suggested a meeting with the neurologists, the ethics committee and the family since the neurologists could much better explain what the undamaged areas of the brain control and if everything else going on with Tommy (infections, swelling, etc) gets better, what we could expect. After talking with them we went back to see Tommy but they weren't done with the new picc line yet. After about 2 hours they were still having problems because of his swelling and they might have to take him down to radiology to have it done.

Well it's been a month and still no word on the case.

11:30A.M. Tommy is getting an EEG Megan said it was ok to go in and see him  The lady that was doing it said  "I would appreciated if you don't talk to him" so we didn't get to see him.
The meeting is still on for 1:00P.M.

12:30P.M.  Dwain Hooten's Mother and sister came by to see us. Was nice to see them. Went in to see If they were ready for meeting.

1:00P.M. Mom, Jo, Karen, Nancy, Tina, Dwayne, Dr. Gene Bolles neurosurgeon,  Olga Second opinion neuro doctor, Dr Campson surgeon, Megan social worker,  Barbara nurse practioner with ethics,  Barbara internal medicine with  ethics, trauma social worker with the ethics, and three others that we didnt catch there names.  Well it started out with if we had any questions.  None at this time. Dr. Bolles put the cat scans up, one was of a normal brain and one was of Tommy's. It was  from Nov. 8, 04. My opinion, it didnt look good.  Not that I knew what to look for but on the comparison of the other.  And you could see the swelling from where the bone flap is out,  its swelling outside of the skull.  Dr. Bolles said that the brain is destroyed on the left side, there is no function, there is some damage to the brain stem.  It was said earlier when they done the bilateral xrays that there was no damage. (Go figure that one uh) As for the lungs Dr Campson said they are doing better, the surgery was successful even though they didnt get to do every thing they wanted before they had to close him up because his other organs were shutting down.  He says there was never any thing said about psueodamonus ( um must of been our imagination again uh) but that there is the MRSA infection.  Campson also said that Tommy has not been off the vent that it has always been with assisted. If they took him off the vent he would die.  (Some one is telling us things that are not true), or they dont know themselves. Olga the second opinion nuero doc, said she took him off the fentinol for several hours and there was not any differance in his state except for his heart rate went up to 151. There was no function, no reaction to pain, no eye movement.  He would be in a vegitative state, Tommy would not speak, no social, no vision and probably won't have a sleep cycle. When asked what recomendations the team had, they said (nothing). Nancy asked how his heart and kidneys were, and they are good.   Dwayne asked about the bone flap and they said that was only cosmedic.  If they put it back in, it would only be to look better and hold the swelling in. They also said some nurses might get squimish. Tommy will not live threw a surgery at this time.  As for the EEG they did this afternoon there wasn't any seizures.  But the EEG was very low and slow.  Jo asked what the part of the brain that is not damaged controls and he said it's like it's severed from the rest of the brain, consider it off on an island by itself (huh?).  Megan said that she is waiting for a call from North Valley to see if Tommy was accepted.  At this point it looks like the only thing to do is hurry and get Tommy into rehab and start moving him around to help fight the infections. Also to give his brain a chance to rest and heal. We all STILL have HOPE and FAITH ! Tommy was very healthy and is a fighter, he is not ready to give up and neither are we.

2:00P.M. Well They have Tommy in Radiology to put the other picc line in.  So we dont get to see him right now.

3:00P.M. Still not back

4:00P.M. Tommy is back and he is doing fine, they didnt get the picc line in, he is too swollen so they just moved it over from where it was.

12:00P.M. Mom talked to Megan.  It is gonna be 2 or 3 weeks before they get the ok for funding on the ventilator to go to North Valley Rehab. What a crock.  SORRY  They are giving Tommy 2 pints of blood because his blood count is down from them draining the pressure sore on the back of his head and where the chest tube was.  His ice blanket is working.  See, I think it does do good to let them see true colors.  Megan said it was ok to have some one come in and massage Tommy or some light physical movement.

2:45P.M.  They are going to change his dressings on his surgery wound, and his head. They drained them today, they didnt look good.  Karen talked to Cam, he is a chiropractor, he might come to see Tommy Tuesday to do some mild physical therapy with him.

Sean said family.  Judge said it would be checked out because he was filing for a public defender.  Sean's next court appearance will be Dec 29, 04  for preliminary disposotion hearing with Judge Harrell.  After court Sean and wife was leaving. They are moving Tommy today to the North Valley Rehab.  At 2:00 he was in the ambulance and on his way to North Valley.  Tommy was very agitated  so they gave him a shot of ativan.  It really knocked him out.  Tina rode in the ambulance with him.  Mom Brandi and Karen followed them.  Every one at North Valley seems to be real nice.  Tommy is in a room with a man name Carlos he is also on a vent. He is aware  of his surroundings.  Mom did a bunch of paper work while they got Tommy settled in.  Lawana and Nancy was also there.

1-08-05

All week-end they will be  assessing Tommy.  Taking pictures of his body.  He had bruises across his lap.  A bad sore on his back side that we were not told about.  The sore on the back of his head was open again, and on his heels.  In one week they will take pictures again to see how they are healing.  Little Tommy and Dylan came to see Tommy and was holding his hand.  Aunt Paula was there also.

1-09-05

Mom is going to get some different curtains for Tommy's room and try to make it a little homier.  Tommy has a bulletin board in his room, if any one would like to send pictures or a card we will hang them. Tommy has his Bronco blanket and is in the process of feeling different textures of different fabrics.  This is part of his recovery.

1-10-05

North Valley is concerned about the bump on Tommys head.  They say it is important for his recoverery that he have a shunt (for draining) and the bone flap but back in.  Mom is trying to get in touch with the neuro surgeons at DGH to see what the procedure is. We were told it wasn't important, was only for cosmetic reasons to but it back in.  North Valley doesn't agree.

1-12-05

They gave Tommy a shower today.  He is so relaxed and looks so happy.

1-13-05

Tommy looks so good, the swelling in his arms, hands, legs and feet is gone.  He looks so relaxed.  They are going to start the process of getting the shunt and the bone flap back in Tommy's head.  This will have to be done at DGH.  North Valley would like for anyone that can or will to please make a cassette tape of there voices and music so they can play them on a daily bases to Tommy.  This is also a part of the process for Tommy's recovery.

Hi I'm back sorry I havent kept you all informed.  Tommy is doing well.  The rehab is going to take Tommy to denver health on Tuesday 02-15-2005 To have the shunt but in him.  This will drain the fluid off his brain and hopefully be able to put the bone flap back in.  Not sure when they will do the bone flap.  Carolyn and every one has made Tommy's room real homey.  Carolyn made blue curtains to go with his bronco stuff.  Joey made a wooden Bronco sign that hangs above his bed.  Joey wants to make a bird house to hang outside his window.  Brandi, Dylan, Tommy, Alycia, and Tony have made him cards and hung them in his room.  There are pictures of all the family.  The incision on Tommy's left side (from the lung surgery) is doing better, and the bed sores.  North Valley is taking pretty good care of him.  I will try to keep you better informed.  Thanks to every one for the prayers.

HAPPY VALENTINES DAY TOMMY

2-15-05

Today Tommy has a doctors appointment to make sure he is infection free.  Doctor gave him a good bill of health, was going to have him come back in 2 weeks to do the shunt.  Mom and Nurse from rehab told the doctor he needed to look at the incision from his lung surgery, it was not healing and that the shunt needed to be done sooner then 2 weeks.  Doctor went out to check and came back and said that they would keep Tommy and put him on the add on list for surgery for the next day.  They will put the shunt in and if it drains fast they will put the bone flap in.  If it does'nt drain fast they will have to but the bone flap in at a later date.  Tommy is in icu because of the ventilator they can not put him in a regular room.  They did a cat scan and Tommy kept coughing so they gave him adivan and fentinol so they could get the cat scan.  They also said that the reason Tommy is on the medication for skitso is when a patient is in acoma they always give it to them.  Good Luck with your surgery Tomorrow Tommy Love you.

2-16-05

Tommy went into surgery at 10:00 A.M.  surgery was 6 1/2 hours.  Tommy done well.  Shunt is in and draining,  it drained alot so they thought they could put the bone flap in. But the bone flap wouldn't fit with out cutting out dead brain tissue and they didnt have previous concent to do it.  Doctors said at a later date they could do a mesh shield for tommy's head.  Vitals are all good he has 7 small incisions and 4 large ones.

Tommy is doing pretty good he really likes shower days.  Little Tommy brings his game boy and moves his dad's fingers on the game boy to play.  The birds really like the bird feeder they are eating the seed fast.  Tommy likes the quiet soft sounds and darker room. Carolyn and Tina is watching a movie with Tommy tonight.  Tommy's bump is all the way down now so the shunt is working.  Because of no bone flap his head is sunk in there. He seems to be loosing a little weight to.  But over all looks good.  Hang in there Tommy we all love and miss you.

Today was court for Sean we had a lot of questions. Debbie from victims advocate met with us before court.  We also talked with Lenny one of the D.A.'s.  They were pretty helpful.  Sean waived his rights for a preliminary hearing.  Now it goes to district court for the arraignment.  It is set for May 2 at 8:30  this date might be changed because there is a conflict of interest with the judge and Sean's lawyer.  They are going to try and change instead of waiting till court and have it continued another time.  Tommy will be getting his diploma from DDRC.  YEAH!!!!!!
North Valley is trying to get Tommy in for the surgery to make the mesh bone flap, the lady that sets up these appt. at denver health is on vacation.  We will let you know as soon as its scheduled.  I went to see Tommy on Monday.  If I rub his left forearm he turns his head to the left.  So I went to the other side and he turned his head to the right when I rubbed his right forearm.  I was pretty amazed.  He doesn't usually do these things when I'm there.  Carolyn always says he does and I havent seen it but he sure did it this time.  So I decided to rub  his feet..  Well at first I dont think he liked it, I thought he was gonna come out of that bed or kick me.  But I kept rubbing his feet and he was enjoying it, I think.  I'm sure one of these days I will find out though.  Love U Tommy.

April 2005

The rehab has assessed Tommy again, things are going well.  Nancy and Mom was at the meeting Nancy has told them that Tommy needs to be sitting up 3 times a week.  Will write more on this as soon as I get all the info.

2-17-06 Tommy still has infections went to the nursing home again.  But is back at the hospital only spends 2 or 3 days there and ends up back in the hospital.  Has broken down skin on tail bone.

The hospital is very upset that Tommys infections come back worse when he goes to nursing home.  The hospital wants Carolyn to take him home.  There is less infection in home nursing care.  Medicaid will set up everything to take him home.  There will be 1 on 1 nuring care 2 / 8 hour shifts This will be a big challenge.  But hopefully it will help Tommy.
